Prices do not include required USA Hockey and CAHA membership costs.
- Mites 8U (born in 2015-2018) - $210
- Squirts 10U (born in 2013-2014) - $300
- Practice Player only- $100
- Pee Wee 12U (born in 2011-2012) - TBD based on players

Payment
Must register and pay in full (or set up a payment plan) by Jan. 15th, 2024.
Pay online or by check to Oak Creek Hockey Association.
*Players cannot practice until registered with Oak Creek Hockey and USA hockey has been completed
Hockey Equipment
Complete gear bags are available - This will be handed out upon payment and registration.
Player Rules and Regulations – Players shall abide by the following rules and regulations. Failure to abide by the following rules may result in loss of membership and/or other disciplinary action at the discretion of the Board of Directors:
Players will abide by the code of conduct, rules, and regulations set forth by USA Hockey, Colorado Amateur Hockey Association, and any league or tournament governing body.
Players will uphold the guiding values of the association as stated in Article 1, Section 4: sportsmanship, respect, integrity, and enjoyment
Players understand that they represent their family, the team, OCHA, and their community.
Players will always behave in a manner that reflects those they represent in a positive light.
Use of tobacco, alcohol, and illegal drugs will not be allowed.
Destruction, vandalism, or stealing in any hockey facility will not be tolerated.
Under the guidance of the Board of Directors, the SafeSport Coordinator, and coaches, players will abide by policies set forth in the OCHA SafeSport Program. Under no circumstances will sexual abuse, physical abuse, emotional abuse, bullying, harassment, or hazing be tolerated.
